Accreditation Responsibility of the Division of Mental Health and Disability Services

The Mental Health, Mental Retardation, Developmental Disabilities and Brain Injury Commission has established the standards in Iowa Administrative Code 441--Chapter 24. Community mental health centers, other mental health service providers, case management providers, and supported community living providers must comply with these standards in order to be accredited to provide those services. The standards form the foundation of accreditation surveys that are completed by Division of MH/DS quality assurance specialists.  All recommendations for accreditation are made to the Commission, which makes all decisions involving issuance, denial, or revocation of accreditation.

Accreditation surveys consist of the Division of MH/DS reviewing policies and procedures, organizational activities and services as determined by the accreditation category; e.g., case management, supported community living, etc. This review may include: on-site case record audits, examining administrative procedures, clinical practices, personnel records, and performance improvement systems; and, interviews with staff and consumers.  Download Application Form 470-3005.  This form is used to apply for accreditation as a service provider under IAC 441-24.  Please contact staff in the Bureau of Accreditation to assist with the processing of the required materials.

During on-site surveys, reviews of files of the individuals served, personnel records, and agency policies and procedures are conducted along with interviews of staff and individuals served.
An agency seeking accreditation may be given "deemed" status if it already complies with the standards set by a national accrediting body or the Home and Community Based Services Waiver for persons with Mental Retardation.  Download Application and Letter of Agreement form 470-3332.  This form is used to apply for deeming for accreditation as a service provider under IAC 441-24.  Please contact staff in the Bureau of Accreditation if you wish to apply for deeming.  They will assist with the processing of the required materials.
